Our mission is to offer quality service for a fair fee, so that all patients can have access to specialist evaluation and treatment. Fees are dependent on what is evaluated & managed during your visit - OR - are based on the amount of appointment time used. This gives you some control over your costs!

FEE FOR SERVICE - This means that although you may first pay out-of-pocket, you can expect to be reimbursed by your Insurance Carrier after a ‘superbill’ is sent.

• Values and discounts are based on standard professional fees with discounts applied to included services.

• MEMBERSHIP PLANS run for 12 months and must be current in order to receive included or discounted services.

• Veterans are eligible for a flat rate of $52.50 per 30-minute appointment - unless you prefer to enroll in one of the other options.

NOT USING INSURANCE AND NOT 100% SERVICE-CONNECTED?

• Consider enrolling in the age-based MEMBERSHIP PLAN

• Consider taking advantage of the 10% uninsured discount on all evaluation and management visits, including Consultation and procedures.

• Consider sending in a Superbill for reimbursement from your insurance.

EVALUATION & MANAGEMENT FEES 

Evaluation and Management (E&M) service fees are requested when you are a new patient - OR - when more time is needed to collect information and/or test for reasons for a NEW or WORSENING condition (“new” conditions are those that have not been formally worked-up or evaluated by Dr. Behling).

NEW PATIENT VISITS

• $85 30-minute evaluation for one (1) minor issue that does NOT require XRAY/MRI/CT or referrals

• $125 45-minute evaluation for one (1) long-standing issue - OR - new issue that does NOT require XRAY/MRI/CT or referrals to sub-specialists

• $165 60-minute evaluation for worsening of 1+ long-standing issues or new issues that require XRAY/MRI or referrals to subspecialists

• $235.75 75-minute evaluation for worsening of 2+ long-standing issues affecting your function or quality of life

ESTABLISHED PATIENT/FOLLOW-UP VISITS    

• $65 30-minute post-procedure re-evaluation (no further procedures needed)

• $85 30-minute evaluation for worsening of one (1) long-standing issue

• $105 45-minute evaluation for one (1) issue not previously evaluated or managed by Dr. Behling - OR - worsening of 2+ long-standing issues

• $143.75 60-minute evaluation for 2+ severely worsening issues with concerns for function, quality of life or safety

A procedure may be performed at any new patient consultation or follow-up visit, if appropriate AND if time allows.

If - at the time of a follow-visit - only a procedure is needed for treatment of a site already evaluated at a prior visit, ONLY a procedure fee will be applied. If there is a significant worsening of a known issue that requires additional investigation or sub-specialist referrals, additional E&M fee will be applied.

Some procedures have flat fees per 15 minutes of time spent during procedure. Some procedures like OMT, ICES® or injections are billed based on number of ‘sites’ treated.
