At each visit, there will be an opportunity to discuss current medication regimen and personal goals.
Patients will be educated on the most effective evidence-based medication options, rotations, and/or regimens (in collaboration with existing providers).
All prescriptions will be provided via electronic prescribing service directly to pharmacy.
To ensure adequate time is available for controlled substance prescription evaluation and consent, a full 30-minute appointment must be scheduled to discuss all the risks, benefits and indications.
Prescriptions for controlled substances will only be provided AFTER the NH PDMP (controlled substance prescription database) is reviewed for non-duplicate and/or inappropriate requests. The patient will be contacted for confirmation of prescription writing once database results have been confirmed.
NO MORE THAN A 5-DAY REGIMEN of controlled substance will be prescribed at one time (unless a confirmed hospice or palliative care patient).
A CONSENT FOR CONTROLLED SUBSTANCES will be required for regimens more than 5-days in duration - OR - more than a total of four 5-day prescriptions (approximately 20 days) in less than six months.
Please note that the State of New Hampshire restricts controlled substance prescriptions to less than 31 days so as to ensure necessity and appropriateness.
CONFIRMATION OF RESULTS FROM URINE DRUG TESTING is also required by the State of New Hampshire prior to providing the initial prescription for controlled substances for MORE THAN 5 DAYS.
Urine drug testing is collected on-site PRIOR to 5+ day regimen of controlled substances - AND - may be required at any time by a Provider for a minimum of twice a year, especially if there are concerns about misuse or inappropriate ‘sharing’ of medications.
North Shore Osteopathic, LLC does NOT offer treatment options for THC/medical marijuana.
