NOW AVAILABLE! All-Inclusive Team-Based

Just to give you every opportunity to be successful in achieving your outcomes, we would like to invite everyone to a new team-based model of care . All patients are eligible to opt-in to this option at any time to achieve more efficient, timely and collaborative care.

CURRENT PATIENTS already managed by at least one of our Providers will NOT need a new consultation - they need only to mention to their Primary Provider their interest in the ALL-INCLUSIVE TEAM-BASED option so that they can be added to the Interdisciplinary Meeting discussions. Once the Team has organized their schedules, the ALL-INCLUSIVE TEAM-BASED FOLLOWUP VISIT will be scheduled. Please note that Team Availability will limit these type of visits to certain days of the week.

As a NEW PATIENT interested in opting-in to the ALL-INCLUSIVE TEAM-BASED care option, please schedule your consultation online - and/or email your chief concerns and how you hope we can help you.
