A Smarter and More Efficient Way to Feel Better: Team-Based Management
Just to give you every opportunity to be successful in achieving your outcomes, we would like to invite everyone to a new team-based model of care . All patients are eligible to opt-in to this option at any time with discounted fees (as our time is streamlined) to achieve more efficient, timely and collaborative care.
CURRENT PATIENTS already managed by at least one of our Providers will NOT need a new consultation - they need only to mention to their Primary Provider their interest in the TEAM-BASED option so that they can be added to the Interdisciplinary Meeting discussions. Once the Team has organized their schedules, the TEAM-BASED FOLLOWUP VISIT will be scheduled. Please note that Team Availability may limit these type of visits to certain days of the week.
As a NEW PATIENT interested in opting-in to the TEAM-BASED care option, please schedule your TEAM-BASED consultation online - and/or email your chief concerns and how you hope we can help you. Team Members will reach out to you for more information before they start organizing their schedules to support your care.
